This is an online e log book to discuss our patient de-identified health data shared after taking his/her/guardians signed informed consent.

Here we discuss our individual patients problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problem with collective current best evidence based input

I've been given this case to solve in an attempt to understand the topic of "patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with diagnosis and treatment plan. 

QUESTION 1: PEER TO PEER REVIEW;
 My peer had made her blog in an organized way, that includes various cases from neurology and a single case on pumonology;
Although she hasn't made her own blog she has very well reviewed  a case pertaining to COPD and associated heart failure ;
The blog would have been better if  she would had reviewed cases related to various departments and that would have made it an extensive blog;
Apart from this I would like to sincerely appreciate her efforts for successfully making the assignment;

I have learnt a lot since i have joined 2 year in terms of patient history taking and providing a provisional diagnosis according to our understanding with the help of the clinical data that is given by our hospital.
I have also done some reading work in various online med resource sites where i could gain quite a knowledge from the humoungous information that is provided on a scientific basis to the public.
